We offer several convenient payment options to make the process simple and accessible for individuals, families, and agencies. All services are private‑pay, and payment is due at the time of service unless otherwise arranged.

Credit Cards

We accept most major credit cards.
A standard processing fee applies to all credit card transactions.

Zelle

Clients may pay securely through Zelle using:

Zelle payments are fast, secure, and have no processing fees.

Cash & Checks

We accept cash and personal checks for all services.

HSA / FSA Cards

Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) may be used for psychological and neuropsychological services.
These accounts allow families to use pre‑tax dollars, reducing out‑of‑pocket costs.

Third‑Party Payors

Schools, attorneys, agencies, and employers may pay directly for evaluations.
Invoices can be issued upon request.


Superbills for Out‑of‑Network Reimbursement

Although we do not bill insurance directly, many clients choose to use a superbill to seek potential reimbursement from their insurance company.

What Is a Superbill?

A superbill is a detailed receipt that includes the information insurance companies require to consider reimbursement. It is not a guarantee of payment, but it allows you to submit your claim as an out‑of‑network provider.

A superbill typically includes:

  • Date(s) of service
  • CPT codes
  • Diagnosis (if applicable)
  • Provider credentials
  • Amount paid
  • Tax ID and NPI

How to Use a Superbill

  1. After your appointment, we can prepare a superbill for you.
  2. Review it for accuracy (dates, fees, and services).
  3. Submit it to your insurance company for potential reimbursement.
  4. Your insurance company may reimburse you depending on your out‑of‑network benefits.

Important Considerations

Before starting services, call your insurance company and ask:
“What is my reimbursement rate for an out‑of‑network psychologist?”

Submitting a superbill means you are choosing to share your diagnosis and treatment information with your insurance company. This becomes part of your permanent medical record.


Family Empowerment Scholarship for Students with Unique Abilities (FES‑UA)

Commonly known as “Step Up” or “Step Up For Students Scholarship”

Families who receive the FES‑UA scholarship may use their funds to pay for psychological, psychoeducational, and neuropsychological evaluations.

How It Works

  1. Contact our office to schedule your child’s evaluation.
  2. We will provide an invoice that meets Step Up requirements.
  3. You upload the invoice to your EMA portal.
  4. Step Up processes the payment directly to Dr. Zaky.
  5. Once payment is approved, your appointment is confirmed.

What FES‑UA Can Cover

  • Psychoeducational evaluations
  • Learning disability assessments
  • ADHD evaluations
  • Neuropsychological testing
  • Other qualifying assessments

This program allows families to access high‑quality evaluations without paying out of pocket.

  • Credit card payments include a processing fee.
  • Zelle, cash, checks, and HSA/FSA cards do not have additional fees.
