We provide comprehensive psychological and neurodevelopmental evaluations, including autism evaluations for children, teens, and adults, ADHD evaluations, learning disability assessments, and forensic evaluations.

Do you diagnose autism in adults?
Yes. We provide full adult autism evaluations using gold‑standard tools and structured clinical interviews. Many adults seek evaluation for clarity, self‑understanding, workplace accommodations, or treatment planning.

Do you accept insurance?
We are an out‑of‑network provider. Many clients receive partial reimbursement from their insurance company. We provide a detailed superbill upon request.

How long does the evaluation take?
The length of an evaluation varies depending on the referral question and the nature of the assessment being requested.

Do you offer telehealth?
Yes. Consultations, interviews, and follow‑up sessions can be completed via secure telehealth. Testing must be completed in person.

How soon can I schedule an appointment?
Availability varies, but we do our best to accommodate clients quickly. Please call or email the office for current scheduling options.

What should I bring to the appointment?
Please bring any previous evaluations, school records, IEPs, medical records, or relevant documentation. These help ensure a thorough and accurate assessment.

Do you evaluate young children?
Yes. We evaluate children ages 2 and up using developmentally appropriate tools and parent interviews.

How do I schedule an appointment?

How early should I arrive for my appointment?
Please arrive about 15 minutes before your scheduled appointment time. This allows time to complete any paperwork and ensures that your appointment begins smoothly and without delay.

I received a letter from SSDD stating I have an appointment. Why?
SSDD schedules independent evaluations to gather additional information about your case. For questions about why the evaluation was requested, please contact SSDD directly.

You may still call our office at 772‑249‑2593 if you need help understanding the appointment process.
Does meeting with Dr. Zaky for SSDD mean he is my doctor?
No. Meeting with Dr. Zaky for an SSDD evaluation does not mean he will become your treating provider. He is serving as an independent evaluator, and the final report is sent only to SSDD.

For questions about your case, please contact SSDD directly.
I was referred by an agency. Can I get a copy of my report?
You must contact the referring agency directly to request a copy of your report. They control the release of records for evaluations they authorize.

Does Dr. Zaky decide SSDI eligibility?
No. Dr. Zaky does not make decisions about SSDI eligibility. He is a neutral evaluator and does not have any role in approving or denying claims.

For case decisions, please contact SSDD.
Should my child take medication on the day of the appointment?
Yes. Your child should take their medication as prescribed unless your prescribing provider instructs otherwise.

Can someone else bring my child to the appointment?
Yes. Another adult may bring your child to the appointment. However, please complete the pre‑interview questionnaire and consent forms beforehand.

Can I drop off my child and leave?
If your child is under 18, a parent or guardian must remain in the building during the appointment.

If your child is 18 or older, they may be dropped off and picked up afterward.

Is there a bus stop near the Port St. Lucie office?
Yes. There is a bus stop directly in front of the medical plaza for your convenience.

What transportation options are available if I don’t have a car?
Several options may be available, including:

Yes, but please call or email the office before stopping by so we can ensure someone is available to receive your documents.
